Midwest Eye Services, LLC d/b/a Bridgeview Eye Partners (“Bridgeview Eye”) understands the importance of privacy. This Website Privacy Notice (the “Website Privacy Notice”) applies to your activity and interactions with us on bridgevieweye.com (the “Services”).  

INFORMATION COLLECTED 

Bridgeview Eye collects information, including Personal Information, directly from individuals and through user interactions with the Services. “Personal Information” means information that can reasonably identify an individual, or as otherwise defined by law. Depending on your interactions with our Services, we collect the following information: 

Contact Information and Identifiers. This information includes name, email address, mailing address, practice address, practice email, and telephone numbers.  

Browsing Information. We collect information about the use of our Services, including IP address, operating system type, browser type, and browsing history.  

Location Information. We collect limited location information, including zip code.  

Information You Provide to Us. We collect information you provide by filling in forms and include in your correspondence. We also collect responses to surveys we may send to you for research purposes.  

USE OF INFORMATION  

Bridgeview Eye uses information for the purposes described in this Website Privacy Notice. We use the information discussed above in a number of ways, including for: 

  • Communications, including responding to your requests and communicating with you; 
  • Providing Services, including managing your preferences on the website, providing you with requested information, and other Service related activities; 
  • Marketing and analytics, such as providing you with tailored content and marketing messages, and analyzing your use of our Services, including your responses to our emails and the pages and advertisements you view; 
  • Managing our business, including operating, evaluating, and improving our business (including developing new products and services and improving existing products and services); 
  • Legal, security, and safety reasons, including the protection of our business and employees; and 
  • Any other purposes, which we will disclose at the time you provide or we collect your information. 

DISCLOSURE OF INFORMATION  

We disclose information for the purposes described in this Website Privacy Notice. We disclose information: 

  • To our service providers and vendors. We transfer all or a portion of your information to our service providers who help us deliver our products and Services to you and for the purposes described in this Website Privacy Notice.  
  • To analytics or tracking technology providers. We disclose your information with analytics and tracking providers such as Google. For more information on these tools, see “Understanding Tracking Technologies” and “Online Advertising” below.  
  • For a merger, acquisition, change in ownership, or reorganization. Information we have collected may be disclosed to a third party in the event of a merger, transfer of ownership or assets, bankruptcy, or other corporate reorganization.  
  • With our affiliates. We transfer the information that we collect to our related affiliates acting on our behalf for the purposes described in this Privacy Notice. 
  • Additional sources. Bridgeview Eye discloses information if we believe disclosure is necessary, including to: 
  • Protect and defend the rights, property, or safety of Bridgeview Eye, our employees, other users of the Services, or the public; 
  • Respond to a legally valid request from a competent governmental authority; 
  • Respond to claims that any content violates the rights of third parties; 
  • Correspond with law enforcement agencies, if we are required to do so; and 
  • Satisfy any applicable law, regulation, legal process, or governmental request. 
  • When you consent. We share information when you give us consent.  
  • For other purposes. We disclose your information for any other purpose disclosed by us when you provide the information.  

Disclosure of Aggregated Non-Personal Information. We may disclose aggregated, non-personal information received from the Services that does not identify any individuals, without restriction.  

SALE AND SHARING OF INFORMATION  

We have not sold (as those terms are defined under applicable data protection laws) your Personal Information for monetary or other valuable consideration; however, our use of cookies and other website tracking technologies may be considered a sale or sharing of Personal Information under these laws, in which case the categories of Personal Information that we have sold or shared include identifiers (e.g., IP addresses) and internet or other similar network activity. Categories of third parties to whom Personal Information is sold or shared include data analytics providers and advertising and marketing providers.  

UNDERSTANDING WEB BEACONS, COOKIES, AND OTHER TRACKING TECHNOLOGIES 

We, or our service providers, and other companies we work with may deploy and use cookies, web beacons, local shared objects, and other tracking technologies for various purposes, such as monitoring our advertising and marketing campaign performance.  Some of these tracking tools may detect characteristics or settings of the specific device you use to access our Services. 

“Cookies” are small amounts of data a website can send to a visitor’s web browser. They are often stored on the device you are using to help track your areas of interest.  Cookies may also enable us or our service providers and other companies we work with to relate your use of our Services over time to customize your experience.  Most web browsers allow you to adjust your browser settings to decline or delete cookies, but doing so may degrade your experience with our Services. 

Clear GIFs, pixel tags, or web beacons – which are typically one-pixel, transparent images located on a webpage or in an email or other message – or similar technologies may be used on our sites and in some of our digital communications (such as email or other marketing messages). They may also be used when you are served advertisements or you otherwise intact with advertisements outside of our Services.  These are principally used to help recognize users, assess traffic patterns, and measure site or campaign engagement. 

Local Shared Objects, sometimes referred to as “flash cookies” may be stored on your hard drive using a media player or other software installed on your device. Local Shared Objects are similar to cookies in terms of their operations, but may not be managed in your browser in the same way.  

ONLINE ADVERTISING 

You will see advertisements when you use our Services.  These advertisements may be for our own Services or for products and services offered by third parties.  Which advertisements you see is often determined using the information we or our affiliates, service providers, and other companies that we work with have about you, including information about your relationship with us.  To that end, where permitted by applicable law, we may share with others the information we collect from and about you. 

Online behavioral advertising (“OBA”) refers to the practice of collecting information from a computer or device regarding a visitor’s web-browsing activities across non-affiliated websites over time in order to deliver advertisements that may be of interest to that visitor based on their browsing history.  Through OBA, we hope to deliver advertisements that are most likely to be of interest to you using information about your other web-browsing activities (i.e. the web pages you visit, search terms you enter, and the way you move through our Services on your computer or device) or your personal profile (i.e. demographic or location information).  Certain companies we work with may set cookies and use related tracking technologies, such as the clear GIFs discussed above, throughout our Services or in our communications to support these OBA efforts.  If you do not want your information collected and used by the companies we work with for OBA purposes, you can opt out of this form of OBA.  There are two ways that you can opt out of OBA from certain ads on third-party websites.  One way is through the cross-industry Self-Regulatory Program for Online Behavioral Advertising managed by the Digital Advertising Alliance (DAA).  To opt out of OBA, click on the following link and follow the instructions: http://youradchoices.com.   Another way to opt out of OBA is by clicking on the Advertising Options Icon featured on certain Bridgeview Eye ads on third-party websites. 

TRANSFER, STORAGE, AND SECURITY OF INFORMATION 

Information Bridgeview Eye collects about you is transferred, processed, and accessed by us in the United States, or another country where we or our service providers operate. Please be aware your information may be transferred to, and maintained on, computers located outside of your state or country where privacy laws may differ from and may not be as protective as those where you live.  

If you are located in the European Economic Area (“EEA”) or other regions with laws governing data collection and use that may differ from U.S. law, please note that we may transfer information to a country or jurisdiction that does not have the same data protection laws as your jurisdiction, as disclosed in this Website Privacy Notice. 

YOUR PRIVACY RIGHTS  

Certain states in the U.S., including California, Colorado, Connecticut, Delaware, Florida, Indiana, Iowa, Montana, Oregon, Tennessee, Texas, Utah, and Virginia provide (now or in the future) their state residents with rights related to their Personal Information. Although some of these rights apply generally, certain rights will only apply to limited individuals or circumstances. To the extent that these laws apply, you may exercise the following rights: 

  • Right to Know and Access Information. Note that much of the information you are entitled to know or access is disclosed in this Privacy Notice. With this said, you have the right to know about our information practices. You also have the right to access the categories of data we collect, with whom we share or sell that information, and, in some cases, what specific Personal Information we associate with you or your account.  
  • Right to Data Portability. If you request a copy of your specific information then we will provide it in an easily accessible format. 
  • Right to Deletion or Erasure. You may request that we delete the information we have collected about you. Depending on the applicable law, in some cases we are required or permitted to retain your information, even if you validly requested we delete or erase it.  
  • Right to Correct Information. You may request we correct or rectify inaccurate information we have collected about you (excluding Iowa and Utah). 
  • Right to Opt out of Targeting Advertising, Sales, or Profiling. You may opt-out of our use of your Personal Information for targeted advertising (excluding Iowa), sales, or profiling in furtherance of decisions that produce legal or similarly significant effects. Please note that we do not use information collected about you for automated profiling at this time. 

Nevada Privacy Rights. Nevada residents have the right to opt-out of the sale of their Personal Information by, emailing us (see our contact information below) and including “NV Sale Opt Out Request” in the subject line. Please note we will take reasonable steps to verify your identity and the authenticity of the request. Please note we do not “sell” data as defined by Nevada law.

Please include your email address, full name, and your specific information about your request(s) and, if applicable, specifically what information you do not want to receive. If you would like to update or correct your email address, street address, or other information with us, please include specific details about the information you wish to have updated or corrected. 

For requests submitted by email, you must provide enough information that allows us to verify your identity. Only you or your authorized agent may make requests regarding your Personal Information.  

We will confirm that we received your request within ten (10) days and will respond within forty-five (45) days of its receipt. If we require more time, we will inform you of the reason and extension period in writing. The response we provide will also explain the reasons we cannot comply with a request, if applicable. For data portability requests, we will select a format to provide your Personal Information that is readily useable and should allow you to transmit the information from one entity to another entity without hindrance. 

We do not charge a fee to process or respond to your request unless it is excessive, repetitive, or manifestly unfounded. If we determine that the request warrants a fee, we will tell you why we made that decision and provide you with a cost estimate before completing your request. 

Appeal. If we deny your rights request, you have the right to request an appeal of our decision. To initiate an appeal, follow the instructions provided in the communication denying your request, or contact us at the information provided in this Website Privacy Notice.  

Non-Discrimination. You have the right to not experience discrimination from us for exercising the rights listed in this section. What we mean by discrimination is denying you access to our Services or limiting the quality of our Services. However, limiting use of, or deleting, your Personal Information may restrict the purposes or uses that rely on that information.  

ADDITIONAL PRIVACY CHOICES 

Messaging and Newsletter Communications. You may control how you receive certain types of communications by unsubscribing within the body of the communication. Note that some messages are required, service-related messages like transaction confirmation messages, legal notices, or updates.  

Cookies and Web Tracking Technologies. You may limit the automatic collection of information by Google, including the collection of Personal Information about your online activities over time and across third-party websites or online Services, by opting-out of such collection. Download the opt-out browser extension, located here, to opt-out of the automatic collection of browsing and device data.  

Do Not Track Signals. CalOPPA requires us to let you know how we respond to web browser Do Not Track (“DNT”) signals. DNT is a privacy preference you can set in your web browser to indicate that you do not want certain information about your webpage visits collected across websites when you have not interacted with that service on the page. For details, including how to turn on DNT, see here. Because there currently isn’t an industry or legal standard recognizing or honoring DNT signals, we don’t respond to them at this time.  

GPC Signal. We respond to the Global Privacy Control (“GPC”) signal. The GPC signal is a browser-level signal that can reflect the desire to opt-out of having Personal Information shared or sold. Some browsers allow transmission of GPC signal. By broadcasting this signal, we will not transmit your Personal Information to third parties who perform analytics services or who advertise to you based on this information.  

Third Party Advertising. We do not control third parties’ collection or use of your information to serve interest-based advertising. However these third parties may provide you with ways to choose not to have your information collected or used in this way. You can opt out of receiving targeted ads from members of the Network Advertising Initiative (“NAI”) on the NAI’s website.  

LINKING TO THIRD-PARTY WEBSITES 

We may provide links to websites or services that are owned or operated by other companies (“third-party websites”).  When you use a link online to visit a third-party website, you will be subject to that website’s privacy and security practices, which may differ from ours.  You should familiarize yourself with the privacy policy, terms of use, and security practices of the linked third-party before providing any information on that website. 

CHANGES TO THE WEBSITE PRIVACY NOTICE 

We may change this Website Privacy Notice from time to time.  When we do, we will let you know by appropriate means such as posting the revised policy on this page.  Any changes to this Website Privacy Notice will become effective when posted unless indicated otherwise.
